Non-rigid 2D-3D image registration for use in Endovascular repair of Abdominal Aortic Aneurysms

چکیده
Ruptured abdominal aortic aneurysms are a relatively common cause of death in the western world. Endovascular repair can be used to reduce the risk of rupture and is becoming the preferred method of treatment in lieu of open surgery. However, intraoperatively only 2D fluoroscopy imagery is available to the surgeon making complex repair difficult. We have been investigating the use of a rigid 2D-3D registration system which enables information from the CT to be overlaid onto the fluoroscopy images during the procedure. The main limitation of this method is that the rigid interventional instruments deform the aorta. This paper investigates the use of manually picked landmarks and the thin plate spline algorithm to deform the CT surface so it more accurately represents the interventional scene. Experiments are carried out on data from eight patients. Results show that the mean error in visceral ostia positions can be reduced from 4.7mm± 2.9mm (mean±standard deviation) to 2.7±1.2mm and 3.2±1.4mm for intra-observer and interobserver guided deformation respectively.

An Aortoenteric Fistula Arising after Endovascular Management of a Mycotic Abdominal Aortic Aneurysm Complicated with a Psoas Abscess
Mycotic aortic aneurysms account for 1–3% of all aortic aneurysms. The management of this disease is controversial. Since open surgical repair is associated with high morbidity and mortality rates, endovascular aneurysm repair is an alternative treatment method with promising early and midterm outcomes, although its long-term durability is unknown.Secondary aortoenteric fistulas may occur iatro...
متن کاملFrequency of Complications Following Endovascular Repair of Abdominal Aortic Aneurysm in Tehran Sina Hospital, 2011-2019
Background and purpose: Abdominal aortic aneurysms (AAA) is a localized enlargement of infrarenal abdominal aorta. Endovascular aneurysm repair (EVAR) is a surgical method introduced for AAA repair. This study, for the first time in Iran, investigated the complications and mortality rate associated with EVAR. Materials and methods: In this cross-sectional study, medical records were reviewed...
متن کاملImproved assessment and treatment of abdominal aortic aneurysms: the use of 3D reconstructions as a surgical guidance tool in endovascular repair.
BACKGROUND Endovascular repair is fast becoming the treatment of choice for abdominal aortic aneurysms in anatomically suitable patients. 3D reconstructions not only aid conventional 2D measurements but also allow further analyses of the vessel anatomy. METHODS Computed tomography scan data for four male patients awaiting endovascular repair were obtained. 3D reconstructions were performed to...
متن کاملGUIDELINES FOR CLINICAL PRACTICE Endovascular stent graft repair of abdominal aortic aneurysms: Current status and future directions
Endovascular stent graft repair of abdominal aortic aneurysm (AAA) has undergone rapid developments since it was introduced in the early 1990s. Two main types of aortic stent grafts have been developed and are currently being used in clinical practice to deal with patients with complicated or unsuitable aneurysm necks, namely, suprarenal and fenestrated stent grafts. Helical computed tomography...
متن کاملMultislice CT angiography in abdominal aortic aneurysm treated with endovascular stent grafts: evaluation of 2D and 3D visualisations
Endovascular repair of abdominal aortic aneurysms has been introduced into the clinical practice for more than a decade and has been confirmed to be an effective alternative to conventional open surgery, especially in patients with co-morbid medical conditions. Helical CT angiography is the preferred imaging method in the follow-up of endovascular repair. Recent introduction of multislice CT sc...
